Please print and use this page as a checklist of items required in the Affiliate Annual Report.
Note:
- DUE DATE for the Affiliate 2011 Annual Report Survey is JULY 15.
- DUE DATE for all other items is JULY 15.
- Items may be sent via email or by U.S. Mail except for dues which must be send via U.S. mail and the online survey which must be sent via the Zoomerang Survey Tool. To send by email, copy and paste web forms into a Word document and fill them out there.
- Please send items individually as you collect them or compile them together in one packet and send them . Please make sure each item is clearly labeled with your affiliate's name and, of course, your name and contact information.

Annual Report Survey
Dues: Please submit your dues and the Affiliate Dues Form via U.S. Mail to the address above.
Roster of Officers: Submit a roster through U.S. Mail, fax, or email. Include the following:
- Name of officer
- Title of position
- Term of position
- Full address (home and school if available)
- Telephone numbers
- Preferred e-mail.
This complete information will help make communication between the affiliate and NCTE efficient and timely. Please be sure to include contact information for your affiliate's liaison officer to NCTE and your affiliate's SLATE Representative to NCTE.
Roster Statement Form: The Roster Statement Form should be attached to or submitted with your roster of officers (by U.S. mail, fax, or e-mail).
Affiliate Directors Form: confirms the affiliate officer(s) who will be attending the Board of Directors Meeting at NCTE's Annual Convention in November---meeting at 5:30 p.m. on the Friday of Convention. All affiliates must be represented at the meeting at least once every three years.
Affiliate Abstract: Please visit the
website list of NCTE affiliates to find your affiliate's information and submit any revisions. This information is used by all NCTE web visitors to locate their local affiliates. Renewing NCTE members and new NCTE members are urged to join thier local affiliates and are sent to this list to locate them.
Policy of Involvement of People of Color: NCTE encourages all affiliates to have a Policy on Involvement of People of Color. If your affiliate has a policy, please include the policy in your annual report. If your affiliate does not have such a policy, please consider adopting one this year. View NCTE's policy as a model.
Policies for Tax-Exempt Organizations: Please note that the IRS requires all
501(c) (3) nonprofit organizations who file using the 990 form to adopt and report on new policies. NCTE joins the IRS in recommending the adoption of these policies by all NCTE affiliates. You'll be asked a question about these policies on the Affiliate 2010 Annual Report Survey.
Budget/Financial Statement: Please submit a copy of your annual budget and/or financial statement. A blank Financial Statement form is available for your use.
Meeting Minutes: Please send a copy your affiliate's meeting minutes for May 1, 2008 - April 30, 2010. You do not need to submit these if your affiliate sends them to NCTE throughout the year. An easy way to ensure that NCTE regularly receives your board minutes is to put the NCTE Affiliate Director on your mailing list to receive them: affsec@ncte.org.
Please attach any affiliate documents that NCTE HAS NOT ALREADY RECEIVED this year, including newsletters, journals, membership brochures, special publications, and conference programs.
NOTE: From time to time, separate surveys will be send to your affiliate's SLATE Representative and Newsletter, Journal, and Web editors of record.

Hopefully, your publication participates in the NCTE Information Exchange Agreement, an agreement enabling participating affiliates, assemblies, and TYCA Regionals to use writing and artwork from one another's publications providing they cite the source of the writing or artwork and send two copies of the publication with the reprinted writing or artwork to the editor of the publication that originally printed the material.
